Identifying Problems — Revision Notes
⚡ 30-Second Revision
Describe the gap before its cause. Check population, period, standard and denominator. Map the process. Seek evidence that distinguishes explanations. Prioritise severity and urgency alongside frequency.
2-Minute Revision
Sixty missed appointments out of 300 is 20%; eighty out of 500 is 16%. The count rises while the rate falls. A review backlog with prompt payment of approved cases points upstream of payment. Attendance decline alone does not prove low motivation. Affected-versus-unaffected route comparisons help test a transport hypothesis without automatically proving causation.
5-Minute Revision
Write a neutral attendance problem statement, then list two competing causes and a measurement that separates them. Calculate both clinic rates and explain which decision uses the count. Draw the scholarship stages and locate the delay from the supplied evidence. Finish by explaining why a rare severe hazard can outrank a frequent minor inconvenience.
