2-Minute Revision

Diagnose the failure before naming the remedy. A backlog can arise from increased inflow, case complexity, insufficient capacity, repeated verification or premature closure followed by rework. Attendance or gross disposal counts cannot separate these causes.

Training will not repair a missing information system by itself; more staff may not resolve a circular approval process. At the same time, a system defect does not erase individual responsibility for concealment or improper favour.

Fair review distinguishes a reasoned good-faith choice from negligence using the information and precautions available at the time. Improve targets by checking actual resolution, reopened files and corrected decisions.

Assign the process change to an actor with the authority and resources to implement it.

Draw a hypothetical benefit file passing through three offices. Identify the missing owner of final resolution and propose a coordination arrangement with real authority. Then design an indicator that would reveal premature closure rather than reward it.
